Much of the action was in the supply closed, or any other available site for a sexual tryst, in this wildly popular soap opera about five first-year surgical residents at Seattle Grace Hospital. The five were insecure Meredith, whose mother was a famous surgeon before being institutionalized with Alzheimer’s; moon-faced Cristina, who was extremely ambitious; blond Izzie, a small-town girl who had paid for medical school by modeling underwear; cocky Alex whose arrogance masked a working-class background; and boyish George, insecure and sometimes bumbling but good-natured about the teasing he received. Their supervising resident was short, sarcastic Miranda, nicknamed “The Nazi” for her imperious ways. The principal attending physicians with whom they worked were handsome, flirtatious Dr. Shepherd, with whom Meredith had a one-night stand the night before she showed up for work and realized he was her supervisor; arrogant Dr. Burke; and fatherly Dr. Webber, the no-nonsense chief of surgery. Joe was the bartender at the Emerald City Bar across the street, where they hung out.

Although there were ER-style medical crises in almost every episode, continuing stories were mostly about the many romantic entanglements. There was Meredith’s ongoing relationship with Derek (“Dr. McDreamy”), which got complicated when his ex, Addison, joined the staff as a neonatal surgeon and tried to win him back. Cristina got hot and heavy with Dr. Burke and Izzie shacked up with Alex, then fell for heart patient Denny, who unfortunately died. Dr. Webber suddenly underwent brain surgery, which led to a power struggle between Burke and Shepherd while he recovered.

In 2006 the proud Burke developed a potentially career-ending hand tremor after a shoot-out in the ER and tried desperately to cover it up. George, having tried and failed to start a relationship with Merredith, found love with tough orthopedic surgeon Dr. Callie Torres. Addison’s former boyfriend, handsome plastic surgeon Dr. Sloan (“McSteamy”), arrived to try to win her back while also hitting on Merdedith and Callie. In early 2007 Dr. Webber announced his retirement, setting off a competition to replace him.

The season four opener found Callie as the new Chief Resident and Richard resuming his role as Chief of Surgery. Among the interns were George, repeating his intern year after failing his exams, and Lexie Grey, Meredith's half-sister whom she has never met. As the season progressed George and Izzie’s affair was revealed and Meredith saw her father for the first time since her stepmother's death. Derek started dating Rose. George and Lexie moved into their new apartment and Lexie stole from the hospital to furnish it.

Season 5 (2008 – 2009) saw Cristina stabbed by an icicle and Derek and Meredith working on their “happily ever after” relationship. Rose stabbed Derek in the hand with a scalpel. Derek tried to move Izzie and Alex out of Meredith's house. Derek received sole credit for the brain tumour study he did with Meredith. The new head of trauma, Owen Hunt, challenged the attending doctor's and the residents' surgical abilities. One of Meredith's oldest friends, Sadie (Melissa George) became an intern at Seattle Grace. Bailey teamed with the new paediatric surgeon, Dr. Arizona Robbins, to save the life of a young patient near and dear to her heart. Izzie told Alex that she was seeing Denny. Derek had a breakdown. Derek finally proposed to Meredith in an elevator. Callie introduced Arizona as her girlfriend to her disapproving father (Hector Elizondo). As Derek and Meredith’s wedding day approached Izzie who had been diagnosed with cancer and was undergoing treatment was discovered to have a second tumour and they gave their wedding day to Izzie and Alex. Izzie underwent surgery to remove the brain tumour. In the season finale George was hit by a bus while saving a woman. The episode ended with George and Izzie dying and meeting in a heaven-like scene, with no indication as to whether they are dead or alive.

The sixth season found Izzie had survived and was mourning the passing of George. Dr. Wyatt. Meredith and Derek finally consummated their wedding. Meredith had to save the life of Lexie’s father whose liver had failed. The Seattle Grace and Mercy West Hospitals merged. Callie’s father arrived with a priest to get her to renounce her homosexuality. April Kepner (Sarah Drew) was fired over the death of a burn victim. Izzie disappeared, sending Alex into turmoil – she returned later with her high school mentor (Joel Grey) seeking treatment for his dementia. Owen recruited a fellow Iraqi War vet, Dr. Teddy Altman (Kim Raver), as the new cardiothoracic Surgeon.

Rob Lowe was originally considered for the role of Derek Shepherd, but passed on it.

Patrick Dempsey auditioned for the role of Dr. Chase on FOX's prime time medical mystery show, "House M.D." (2004), before taking the role of Dr. Shepherd.

The show's title is a reference to the famous anatomy book "Gray's Anatomy of the Human Body" by Henry Gray.
